The Prisons and Probation Ombudsman has launched an inquiry into the death of a 37-year-old man at HMP Forest Bank in Salford.
An investigation has been opened following the death of Anthony Stephen Copeland, a 37-year-old man who died in custody at HMP Forest Bank on 21 August.
The Prisons and Probation Ombudsman (PPO) confirmed that an inquest has been opened and adjourned, and that a formal inquiry into the circumstances surrounding Mr Copeland’s death is now underway.
HMP Forest Bank is a Category B men's prison located in Pendlebury, Salford. It is privately operated by Sodexo Justice Services. The prison has previously faced scrutiny following multiple inmate deaths and reports raising concerns about safety and staffing.
Mr Copeland's details, including his full name, age, and date of death, were published on the PPO’s official register of deaths in custody. At this stage, the cause of death has not been made public, and no further details have been released by the authorities.
The Ministry of Justice and Sodexo have not issued a statement. Greater Manchester Police has also not confirmed whether any separate investigation is being conducted.
The PPO investigates all deaths in custody across England and Wales, including those occurring shortly after release, as part of its statutory duties.
